Private Insurance
Your private insurance may have chiropractic benefits. Policies can differ vastly in their coverage. We will gladly attempt to look up your coverage when you come in but ultimately it is your responsiblity to know your benefits.
You may have a Co-pay, Co-insurance and/or a Deductable.
We participate in the major plans such as Independent Health, BlueCross/Blue Shield, Univera, United Health, Medicare and most other major insurers. Dr. Hopkins is a participating provide for Empire as well.
Co-Pays due on the day of treatment services. We cannot bill Co-Pays. (A $5.00 fee will be applied if you request to be billed)
